Thyroid Transcription Factor-1 (TTF-1) is a nuclear protein encoded by the *NKX2-1* gene, belonging to the NKX2 family of transcription factors. Primarily expressed during embryonic development, TTF-1 plays a critical role in the morphogenesis and differentiation of the thyroid, lung, and ventral forebrain. In adults, its expression is largely restricted to thyroid follicular cells and pulmonary epithelial cells, particularly type II pneumocytes and Clara cells.
TTF-1 antibody is widely used in diagnostic immunohistochemistry (IHC) to identify tumors of thyroid or pulmonary origin. It serves as a key marker for lung adenocarcinomas (positive in ~70-80% of cases) and thyroid carcinomas (papillary and follicular subtypes). In contrast, it is typically negative in squamous cell carcinomas and mesotheliomas, aiding in differential diagnosis. The antibody detects TTF-1 protein via nuclear staining, with clones like 8G7G3/1 and SPT24 commonly employed.
While TTF-1 positivity strongly suggests pulmonary or thyroid lineage, exceptions exist. For instance, some neuroendocrine tumors (e.g., small cell lung carcinoma) may express TTF-1. whereas extrapulmonary adenocarcinomas (e.g., gastrointestinal or breast) are usually negative. Clinically, TTF-1 is invaluable in determining the origin of metastatic tumors and guiding therapeutic decisions. Its utility is often enhanced when combined with other markers like Napsin A for lung adenocarcinoma or Thyroglobulin for thyroid tumors.
