The A4GNT antibody targets the α1.4-N-acetylglucosaminyltransferase (A4GNT) enzyme, a glycosyltransferase critical for synthesizing unique O-glycan structures in the gastrointestinal tract. A4GNT catalyzes the transfer of GlcNAc residues via α1.4-linkages to core 1 mucin-type O-glycans, forming terminal α1.4-GlcNAc-capped structures. These glycans are predominantly expressed in gastric gland mucous cells and play a key role in mucosal defense.

A4GNT's activity is particularly associated with the formation of the "glycoform" recognized by the pathogen Helicobacter pylori, which binds to α1.4-GlcNAc residues during gastric colonization. Reduced A4GNT expression has been linked to gastric pathologies, including chronic gastritis and gastric cancer, as impaired α1.4-GlcNAc production compromises mucosal barrier function.
