STAR (Steroidogenic Acute Regulatory) antibodies are essential tools in studying the regulation of steroid hormone biosynthesis. The STAR protein, encoded by the *STAR* gene, facilitates the rate-limiting step in steroidogenesis: transporting cholesterol across the mitochondrial membrane to the inner mitochondrial space, where it is converted into pregnenolone by CYP11A1. Discovered in the 1990s, STAR is predominantly expressed in steroidogenic tissues, including the adrenal glands, gonads, and placenta.

Structurally, STAR contains a cholesterol-binding domain and undergoes phosphorylation to activate its function. Mutations in the *STAR* gene cause congenital lipoid adrenal hyperplasia (Lipoid CAH), a severe disorder characterized by impaired steroidogenesis, leading to adrenal insufficiency and ambiguous genitalia in genetic males.
